Educational Qualification
- Minimum Qualification: MBBS degree.
- Registration: Must be registered under Assam Medical Council, 1960.

- Proof of Age: Must be based on Class X or XII Admit Card/Pass Certificate/Mark Sheet issued by a recognized board where Date of Birth (DOB) is clearly mentioned.
- No other document will be accepted as proof of age.
Persons with Benchmark Disabilities (PwBD)
- Must possess a valid Benchmark Disability Certificate issued by a competent authority.
- Upload: Certificate in prescribed format.
Candidates Working in Government Services (Assam)
- Must upload a certificate of service issued by the competent authority under whom the candidate is currently working.
- If applying for pay protection or service continuation, a No Objection Certificate (NOC) must also be uploaded.
Declaration on Small Family Norms
- As per Government Notification No. ABP.69/2019/17, dated 06.11.2019:
- Candidates must upload a declaration in Form-A regarding compliance with Small Family Norms at the time of application.
- Upload: Duly filled and signed Form-A as part of the application.
